Fill out this form to request the ChivaGirls at your event.

Please review the following guidelines:

A minimum of 4 ChivaGirls required for all dance performance requests (Client must provide sound system with CD player and a seperate private area for ChivaGirls to warm up.
If you wish to cancel a ChivaGirl appearance, it must be received and confirmed in writing 72 hrs. prior to scheduled event. Cancelled appearances with out confirmed notice will be charged full appearance fee.
Events requested further than 30 miles in distance from the Home Depot Center may be charged an additional transportation fee.
Media related requests and/or continued usage of print, video, film or commercial footage is subject to seperate fees & rates.

Reminder: The use of Chivas USA logos, trademarks and/or player photos must be approved in advance in writing by Chivas USA organization. Organizations can not advertise player appearances until they have been confirmed by the Chivas USA’s Community Affairs department.
